APPLICATION FORM AND GENERAL EXPLANATIONS

 

Relevant persons owning personal data (“Relevant Person”), have the right to apply to Agreka Bilimsel Doğal Yaşam Ürünleri San. Ve Tic. Ltd Şti.’ne (“Company”), To exercise the rights specified in Article 11 of the Law on the Protection of Personal Data No. 6698 (“KVKK”). The said rights of the Related Person are as follows;

 

• Learning whether your personal data is processed or not,
• If your personal data has been processed, requesting information about it,
• Learning the purpose of processing your personal data and whether they are used in accordance with the purpose,
• Knowing the third parties to whom it has been transferred in the country or abroad,
• Requesting correction of personal data if it is incomplete or incorrectly processed,
• Requesting the deletion or destruction of your personal data within the framework of the conditions stipulated in the KVKK,
• Requesting notification of the actions taken pursuant to your rights in the form of correction, deletion and destruction mentioned above, to third parties to whom personal data has been transferred,
• Objecting to the emergence of a result against you by analyzing your processed personal data exclusively with automated systems,
• Requesting the compensation of your damage in case you suffer damage due to the processing of your personal data in violation of the relevant legislation.

 

1. Explanations on the Application Method

In order for our company to respond to the applications made by the Relevant Persons within the scope of Article 11 of the KVKK, the application procedure must comply with the provisions of the Communiqué on the Procedures and Principles of Application to the Data Controller (“Communiqué”). Our company has prepared this application form in order to facilitate the application in the manner stipulated in the legislation. It should be emphasized that, pursuant to paragraph 2 of Article 4 of the Communiqué, Relevant Persons can benefit from this right provided that they make their applications in Turkish.

Relevant persons can apply to the Company through the following methods:

 

• Personal application,

• Via notary public,

• By sending an e-mail to the registered e-mail address of the Company by signing with a secure electronic signature or mobile signature,

• By sending an e-mail from your e-mail address previously reported to our company and registered in the system.

    

* Relevant Persons who will apply for "Personally Written Application" are required to submit documents proving their identity. In this context, we kindly request you to submit only the front side of the ID photocopy (without showing blood type and religion) along with the application form for the personal written applications that will be made to address Saray Mah. Dr. Adnan Buyukdeniz Cad. (OryaPark) No: 3 A Da: 120 Ümraniye / İSTANBUL. 

** We kindly request that "Personal Data Protection Law Relevant Person Request" be written in the notification envelope of the application form or in the subject of the e-mail.

5.  Responding to Your Applications by the Company

The Company always reserves the right to request additional information and documents proving your identity in order to prevent the unlawful sharing of your personal data with third parties and to ensure the security of your personal data.

Relevant person requests will be finalized by the Company as soon as possible and within 30 (thirty) days at the latest, depending on the nature of the request, pursuant to Article 13/2 of the KVKK. Our answers will be delivered to you in writing or electronically in accordance with the provisions of Article 13 of the relevant KVK Law. 

 

I accept, declare and undertake that I know that the personal data I shared with the Company in this application form is correct and up-to-date, that I have not made any unauthorized application, otherwise any legal and/or criminal liability that may arise will belong to me.
